RMA Request Form

This form is for product returns only. For warranty claims, please use the Warranty Return Form.
The PDF version of the form is still available for those that prefer to use it.
Product is returnable for a 25% restocking fee under the following conditions:

• Purchased within 12 months of return date, referencing original purchase order number
• Active inventory in current catalog (WSL, CLR, CD, OBS components excluded)
• Remains in the original, unmarked, unopened packaging
• New, clean, and re-saleable condition
• Returned prepaid to HeatLink within 14 days of RMA issuance

Notes:

1. If the product received is unsatisfactory, or deemed otherwise unsaleable, it will be disposed of or returned at the returnee’s discretion & cost.
2. Any exceptions to the above criteria requires Sr. Management approval.
