This form is for product warranty returns only.

Instructions:

1. Fill out this form and click "Submit". Fields marked with a star (*) are required. You will receive an email copy of your submission.
2. HeatLink will issue an RMA within 2 business days.
3. The RMA will include instructions on what to do with the product. If the product is to be returned, it must be returned within 14 days.
4. The product will be tested and you will receive the warranty report within 2 weeks of HeatLink receiving the parts.
